EFFECT OF SAPTAPARNA PICHU IN DUSHTA VRANA

Abstract

Vrana management requires a skillful and keen observance to bring the best positive outcome of any surgery.  Due to the lack of proper management, Vrana gets changed into Dushta Vrana. So new approach is continuing to develop for the Vrana management. The aim of this study was to evaluate the efficacy of Saptaparna Pichu (local application) in the management of Dushta Vrana. After approval of IEC and registration under clinical
trial registry of India, 30 patients suffering from Dushta Vrana were registered in the trial. Raskriya of the Saptaparna bark was prepared and then coated over gauge to make into the form of Pichu.  Local application of Saptaparna Pichu was done after proper cleaning followed by sterile bandaging at every third day. Assessment was done based on the Subjective (Pain and Itching) and Objective (Tenderness, Size, Smell, Discharge, Floor/ Granulation) parameters at an interval of 15 days. Total duration of treatment was 6 months (maximum) and follow up was done at interval of 15 days for a period of two months after completion of therapy.

All information which was based on various parameters were gathered and statistical
analysis was carried out in terms of Median, First Quartile (Q1), Third Quartile (Q3),
Wilcoxon test and finally result were incorporated in term of probability ‘P’.

The clinical symptoms in wounds showed statistically significant reduction following treatment. The overall result shows that 66.7% were cured completely, 23.3% were markedly improved, 6.66% moderately improved and only 3.33% of patients shows unchanged results. Since observations were on ordinal scale (gradations) with sample size of 30, Wilcoxon Signed Rank Test were used to test efficacy. On calculating the values, we found P-Value for all parameters was less than 0.001. Hence, it was concluded that, effect observed in all parameters was highly significant.

This clinical study concludes that Saptaparna in the management of Dushta Vrana possess excellent qualities like Shodhana, Ropana, Vedana-sthapana, Shoth-har, Lekhana, Shoshana, etc. Also, in statistical analysis, Saptaparna pichu showed highly significant results. Moreover, this therapy was found to be very cost effective, can be easily prepared with no side effects in Dushta vrana management.
